Chrysalis Dublin 1 & City Clinic

Chrysalis is located in the North East Inner City (NEIC) . Our team consists of 11 Case Managers who work from the Social Inclusion Health Hub in Summerhill who are placed to work alongside individuals connected with the (NEIC). The Case Management team work as part of a multidisciplinary team to provide our service users with the most appropriate care. We are also currently working with a diverse number of services in the area allowing us to engage with as many people as possible.
We are utilising the initial and comprehensive needs assessments currently being piloted by the North Inner City Drug & Alcohol Task Force and developing a structured care plan specific to the service users’ needs. This ensures a consistent approach to Case Management, putting the service users at the centre of all decisions and enables better interagency working on behalf of the service user.
City Clinic Amiens Street Team
Our team of Case Managers, who work from Amiens Street City Clinic, provide in reach formal Case Management to the individuals who access City Clinic. They will work alongside the individual to assist them to address ongoing problems relating to substance use, housing, medical supports.
Community Team
As part of service delivery the Case Management team are engaged with the Addiction Assessment Unit located in Summerhill, Dublin 1. The Addiction Assessment Unit provides access to a doctor, nurse, counsellor and Case Manager to ensure effective and coordinated approach to care. Referrals are made through the service users GP.
Probation Team
We have allocated Case Managers who work alongside the North Dublin Probation Team. The Case Managers who work with the Probation Team aim to identify clients at a social disadvantage as a result of substance misuse and/or who is experiencing homelessness.
Dual Diagnosis Team
Do you have concerns about mental health and addiction? You can access Chrysalis Case Management team who are in the Summerhill inclusion Health Hub. Please ask to speak to one of our Case Managers who will arrange a referral to the service. Our case managers work alongside psychiatric services to ensure an inclusive and whole approach to care. This service operates once a week on a Thursday.
The process:
Once we have received a referral you will be asked to attend an assessment with a case manager. This process will help determine what supports you require. If you require access to dual diagnoses support your case manager will highlight this with the psychiatric doctor. From here you will attend an appointment with the psychiatric doctor and your case manager will keep you informed of appointments and continue to work with you in the capacity of Case Management while working on other aspects which you require support.
Criteria of entry:
- 18 years
- History of addiction
- History of mental health
- Address in the NEIC
- Referrals from other services in the NEIC accepted
Drug Treatment Court Team
We have allocated Case Managers who are allocated to work within the Drug Treatment Court. The Case Managers work as part of a multidisciplinary team to address ongoing issues which are presenting for client such as substance misuse, mental health, housing and social needs which are unmet.
PMVT Barrymore Residential Stabilisation Unit Team
Our team are the allocated Case Management team for clients living in the Dublin 1 (NEIC) and Dublin 7 who are seeking to access PMVT residential stabilisation program. Our team of Case Managers actively work with the client priorto admission, and post admission to Barrymore. During this time we work with the client to identify barriers to accessing the service in addition to focused key working sessions to ensure the readiness to access residential stabilisation.
Entry criteria for service users:
- Must have a connection to the North East Inner City, Dublin 1
- Must be over 18
- Should not have an effective addiction care plan/case manager already in place in another agency
- Should have multiple and complex needs
- Should want to have a care plan and avail of case management
Working hours:
Monday to Friday, 9am to 5pm
